Copay, coinsurance, or deductible amounts

Any referral or pre-authorization requirements
Out-of-network coverage if you plan to submit a superbill
Tip: Write down the representative’s name, reference number, and date of your call to keep a record.
Step 2: Share Your Insurance Information With Us
After confirming coverage, please email us:

Your full first(s) and last name(s)
Your date of birth(s)
Your home address(es)

Step 3: Payment Options
We provide superbills for self-pay reimbursement
We accept bishop or clergy payment
If you’re unsure whether to use insurance or self-pay, we can guide you
Step 4: Schedule a Session
Once your benefits are verified, we can schedule your free 20-minute Growth Pathway session to discuss your goals and next steps in therapy.

Individual Therapy: $175 per session
Couples Therapy: $200 per session

Insurance Clients
For insurance, rates and out-of-pocket costs depend on your specific plan. Your copay, coinsurance, and deductible determine your financial responsibility. We verify benefits before your first session so you know exactly what your plan covers.
Insurance sessions are scheduled for 53 minutes, in line with insurance requirements for time-based psychotherapy codes (90837 and 90847). Self-pay sessions are generally 50 minutes.
Copays: Collected after the session.
Coinsurance: Determined after insurance processes the claim; you pay any remaining balance.
Deductibles: Your responsibility until met; coverage typically increases after deductible is reached.
All amounts are based on your insurance plan, not your self-pay rate.
We submit claims on your behalf after each session. Only time actually provided is billed. Processing times vary by insurer.
Coverage depends on your plan. Some insurers cover only telehealth, some only in-person, and some both. We verify benefits before your first session.
We are credentialed with:
Aetna, PEHP, Regence Blue Cross Blue Shield of Utah, Select Health, United Health Care
Notes:
We do not accept Medicare.
Coverage, copays, and deductibles vary by plan.
All benefits must be confirmed with your insurance company.
Yes:
HSA/FSA for therapy sessions
Bishop or clergy pay for church-supported counseling
Private pay for all other sessions
We value your time and ours. Please arrive on time for your scheduled session so you receive the full benefit and other clients are not delayed.
Cancellations and missed appointments:
At least 24 hours’ notice is required if you need to cancel or reschedule.
Late cancellations or no-shows may be charged the full session fee.
Insurance does not cover missed-session fees.
Each client receives one late-cancel/no-show waiver per calendar year. After that, fees apply fairly and consistently.
Late arrivals:
Sessions will end at the scheduled time if you arrive late; lost time cannot be added to future appointments.
Self-pay clients are responsible for the full session fee, even if late, to protect your reserved time.
Insurance clients will be billed for the actual time provided, and balance billing for missed time is typically not allowed under insurance contracts.
Why this policy matters:
Having a clear, written policy protects both you and your therapist.
This policy is reviewed during your initial visit along with informed consent, financial responsibility, and billing procedures.
Respecting scheduled times ensures fairness to all clients and supports continuity of care.
