Before your first session

How to Verify Your Insurance

We want to make insurance verification simple so you can focus on your healing. Follow these four steps to confirm your coverage before your first session.

1

Call your insurance provider

Call the customer service number on your insurance card and ask the following:

Is Joshua Otani, LMFT at Big Valley Therapy in-network?

  • Provider name: Joshua Otani, LMFT
  • Practice: Big Valley Therapy
  • Address: 59 W 9000 S, Ste 4, Sandy, Utah 84070
  • Phone: (801) 901-0559
  • NPI: 1528617800

Also ask about:

  • Coverage for individual therapy (CPT 90837 – 53 min)
  • Coverage for couples therapy (CPT 90847 – 53 min)
  • In-person vs telehealth coverage and rates
  • Copay, coinsurance, or deductible amounts
  • Number of sessions allowed per year
  • Any referral or pre-authorization requirements
  • Out-of-network coverage if submitting a superbill

Tip: Write down the representative's name, reference number, and date of your call to keep a record.

2

Share your insurance information with us

After confirming coverage, please email josh@bigvalleytherapy.com with:

  • A clear photo of the front and back of your insurance card
  • Your full name(s) and date(s) of birth
  • Your home address
  • Your phone number and email

We will verify your benefits and provide a clear summary including copay, coinsurance, deductible, session limits, and telehealth vs in-person coverage.

3

Review your coverage summary

Once we verify your benefits we will send you a clear summary of your coverage so there are no surprises at your first session — including your expected out-of-pocket cost per session.

4

Schedule your first session

Once benefits are verified, we schedule your free 15-minute consultation to discuss your goals and confirm therapy is a good fit before committing to ongoing sessions.

Billing details

Billing & Policy Questions

How are copays, coinsurance, and deductibles handled? +

  • Copays are collected after the session.
  • Coinsurance is determined after insurance processes the claim — you pay any remaining balance.
  • Deductibles are your responsibility until met — coverage typically increases after your deductible is reached.
All amounts are based on your insurance plan, not your self-pay rate.

How are insurance claims submitted? +

We submit claims on your behalf after each session. Only time actually provided is billed. Processing times vary by insurer — most claims are processed within a few weeks.

Does insurance cover telehealth sessions? +

Coverage depends on your specific plan. Some insurers cover only telehealth, some only in-person, and some both. We verify this before your first session so you know exactly what is covered. Some plans only cover in-state telehealth — we will confirm this for you.

What is your cancellation and late arrival policy? +

Cancellations: At least 24 hours' notice is required to cancel or reschedule. Late cancellations or no-shows may be charged the full session fee. Insurance does not cover missed-session fees.

Each client receives one late-cancel / no-show waiver per calendar year. After that, fees apply fairly and consistently.

Late arrivals: Sessions end at the scheduled time if you arrive late — lost time cannot be added to future appointments. Self-pay clients are responsible for the full session fee. Insurance clients are billed for actual time provided.

This policy is reviewed during your initial visit alongside informed consent, financial responsibility, and billing procedures.
Do you offer payment plans or a sliding fee scale? +

No. Big Valley Therapy does not offer payment plans or a sliding fee scale. Session fees are consistent for all clients.

How can I contact billing? +

We are happy to answer any questions about insurance, session fees, or payment options.
